BENEFITS OVERVIEW
MEDICAL PLANS There will continue to be two medical plan options: Base Plan and Buy-Up Plan. Both medical plans will continue to be administered by Trustmark/ELAP.
(NEW!) DENTAL PLANS Starting in 2024, the two dental plans will be administered by Lincoln Financial. These plans provide the same benefits as the 2023 Delta Dental plans.
VISION PLANS The vision plan will continue to be administered by NVA.
SPENDING ACCOUNTS The flexible spending accounts will continue to be administered by Ameriflex.
LIFE AND DISABILITY The life and disability plan will continue to be administered by The Hartford.
ENROLLMENT CONSIDERATIONS
Benefit elections you make during open enrollment will become effective on 1/1/24. If you have questions about any of the benefits stated in this guide, please do not hesitate to reach out to Sharon Lockwood, or The Graham Company Service Line at 1-888-842-1488. Open enrollment is the period each year when you can make changes to your benefits. This guide will outline benefit options that are available to you and your family.
WHO IS ELIGIBLE?
All full-time employees working 30 or more hours per week are eligible for health insurance, dental insurance, vision and the Flexible Spending Account.
DEPENDENT COVERAGE
In addition to electing coverage for yourself, you can elect to cover your eligible dependents. Your eligible dependents include your legal spouse, and your dependent children. Please refer to your plan documents for a complete list of dependent eligibility rules. New enrollees will need to provide appropriate documentation (marriage certificate, birth certificate).
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period.
Qualifying events include:
- Marriage or Divorce
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
- Entitlement to Medicare or Medicaid
