YOUR NEW ID CARDS EFFECTIVE June 1, 2021

ID CARD NOTICE - USPS DELAY

All Skill Survey members will receive new Medical and Dental ID cards for the 2021-2022 plan year, even if you stay on the same plan. Vision cards are not provided; however, they can be printed per the instructions below. Due to UHC internal system upgrades and the current USPS delays, UHC is notifying all members that your new ID cards may be delayed. Below is a summary of expected shipment dates, as well as a temporary ID workaround, so that you can continue to seek care as needed beginning 6.1.2021.

• Medical – anticipated post mark date May 27th – updated ID cards will show in myuhc.com for members by 6/1/2021. • Dental – anticipated post mark date June 8th - updated ID cards will show in myuhc.com for members by 6/11/2021. • Vision – there are no plastic vision cards – members can print a copy if needed with the instructions HERE or use their Social when visiting the eye doctor.

Below are some helpful notes for you to reference, while you await to receive the hard copy of your ID card. 1. Members can use their medical ID card to seek care at a Dentist while waiting for their physical ID cards to arrive and the electronic versions on myuhc.com to show. The Alternate ID number is the same for medical and dental. 2. Members can also pull up their ID cards and review their benefit information on the UnitedHealthcare App in addition to the myuhc.com portal. (Note, this is a great feature if you are ever at the docotr's office and forget your ID card!) Simply show the provider your phone with the image of the card and you are on yourway.

3. You can also share your ID cards from the UHC app. It will reformat the cards into PDF versions and you can text or email the card directly from the app if needed.
