Skill Survey is thrilled to be able to offer enhanced UHC medical benefits to employees for the June 1, 2021 - May 31, 2022 plan year. The HDHP HRA plan will remain at NO COST to all Skill Survey employees. The Choice Direct Contributions are increasing substantially. Please reference the employee contributions page for additional information.

Please note, all members will receive a new ID card for the 2021-2022 plan year, even if you stay on the same plan.

NO COST OPTION 1:

HDHP HRA

The HDHP HRA plan is offered at NO COST to all Skill Survey employees. The HDHP HRA plan has an enhacement for the 2021-2022 plan year. Outpatient Surgery in a hospital setting is now covered at 100%, after the deductible has been satisfied. The HRA plan will remain with a $3,000 single / $6,000 family deductible where the first portion of those potential costs are funded by SkillSurvey. If you enroll in the HRA HDHP plan, employees will have access to $2,250 single and $4,500 family in HRA funds provided by SkillSurvey. The funds will be loaded onto member specific credit cards and can be used for qualified medical and prescription expenses that are applied to the deductible. When you take into consideration the employer funded HRA, the net employee deductible is $750 single; $1,500 family.

OPTION 2:

CHOICE DIRECT

The Choice Direct plan is remaining the same for the 2021-2022 plan year, with the exception to prescription drug copayments. The copay for generic medications has decreased to $10 copay, while the copay for non-formulary medications has increased to $60 copay. Please note, this is the more expensive plan option and contributions are increasing for the 2021-2022 plan year.

HOW TO USE YOUR HRA FOR MEDICAL

Present your UHC Card

Complete your visit with provider

Provider will submit claim for processing visit

UHC will adjust the claim and prepare the EOB (Explanation of Benefits)

You will receive an EOB and and invoice from your provider

Pay the patient responsibility from your HRAaccount

Pharmacist will submit claim at point of service

Pay the Rx cost from your HRA account at the pharmacy

UHC members can create an account on myUHC.com or download the MyUHC APP to take advantage of numerous resources and become more involved in the health plan. Plan Information is available at your fingertips and can be accessed at any time. Using your Account, you can:
