INTRODUCTION

Understanding your healthcare options and determining the best plan options that fit your needs should be a high priority during the Annual Open Enrollment Period. We understand that navigating through your benefits package can be complex and wish to offer as much support and valuable information as possible.
2021 BENEFITS HIGHLIGHTS
UHC MEDICAL: SkillSurvey continues to offer our employees a platinum-rated, no-cost option for medical coverage. This premium plan is the HDHP with HRA. New this year, employees will have the option of the traditional Choice Direct plan; however, the employee contribution structure has substantially changed. BOTH plans offer identical prescription formularies and ONLY offer in-network coverage. All members will receive a new ID card for the 2021-2022 plan year, even if you stay on the same plan.
OPTION 1: No-Cost HDHP HRA: The HDHP HRA Choice Plus plan has one major enhancement/advantage for the 2021-2022 plan year, Outpatient Surgery in a hospital setting is now covered at 100% (previously 80%), after the deductible has been satisfied. OPTION 2: Choice Direct: The Choice Direct plan has some changes to the prescription drugs co-pay structure. The copay for generic medications has decreased, while the copay for non-formulary medications has increased. Unlike the No-Cost HDHP HRA plan, outpatient surgery in a hospital setting continues to have a benefit of only 80%. Choice Direct contributions have increased for the 2021-2022 plan year. UHC DENTAL: Your dental benefits are not changing for the 2021-2022 plan year and will continue to be offered through UHC. You may visit any dental provider with this plan; however, you will receive a greater discount by visiting a UHC In-Network participating provider. Please note, your dental deductible and out-of-pocket maximum will now reset on a PLAN YEAR basis (6/1-5/31) as opposed to CALENDAR YEAR (1/1-12/31). Additionally, all members will receive a new ID card for the 2021-2022 plan year.
UHC VISION: Your vision benefits and rates are not changing for the 2021 plan year and will continue to be offered through UHC. Please note, you may visit any eye doctor with this plan; however, you will receive a greater discount by visiting a UHC Network participating provider. Note, all members will receive a new ID card for the 2021-2022 plan year.
LINCOLN FINANCIAL DISABILITY AND EMPLOYEE ASSISTANCE PROGRAM: SkillSurvey provides all full time employees with Short Term Disability and Long Term Disability at no cost to you. Additionally, you have access to a confidential, 24/7 Employee Assistance Program (EAP), available at no cost.
CLARITY SPENDING ACCOUNTS: SkillSurvey provides you with the opportunity to participate in several tax-advantage spending accounts outlined below. Please note, if you wish to enroll in a spending account for 2021, you must actively make your benefit election, otherwise your 2020 elections will be forfeited and will NOT automatically rollover.
1. Medical Flexible Spending Account
- NEW! This year, your FSA Rollover amount will NOT be limited to $550. Any remaining funds as of 5/31/2021, will rollover into the 6/1/2021 plan year.
2. Dependent Care Spending Account
- NEW! For the 2021 calendar year you may elect up to $10,500, you must carefully choose your election as you may not be reimbursed for more than $2,500 single / $5,000 family in the 2022 calendar year.
ENROLLMENT CONSIDERATIONS
Elections you make during open enrollment will become effective on June 1, 2021. NEW! This year, SkillSurvey will be using EMPLOYEE NAVIGATOR to complete all of your benefit enrollments. Open enrollment is the period each year when you can make changes to your benefits. This guide will outline benefit options that are available to you and your family.
WHO IS ELIGIBLE?
If you work 60 or more hours per pay, you are eligible for the following: New Hires - Medical, Dental and Vision date of hire. - Short-Term Disability and Long-Term Disability Insurance date of hire. Employees who Increase Biweekly Hours to 60 or Above If your regularly scheduled biweekly hours increase from less than 60 to 60 or more, you are eligible to enroll in all benefits listed above on the 1st of the month following your change of status provided you have met the waiting periods outlined above for each benefit. Employees who Experience a Qualified Change of Status Qualified Changes must be reported to Human Resources within 30 days of the event. Family Members of Employees who work 60 or more hours per pay The following family members are eligible for Medical and Vision coverage: a legal spouse and children up to age 26. The following family members are also eligible for Dental: a legal spouse and unmarried children up to age 26. (You are required to provide appropriate documentation for all dependents enrolled in the plans (i.e. birth certificate or a marriage certificate, etc.)
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period—each June 1st.
Qualifying events include:
- Marriage or Divorce
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
- Entitlement to Medicare or Medicaid
