SkillSurvey will continue to offer dental and vision through UHC. Both the dental and the vision plan is remaining the same.
DENTAL
DENTAL PLAN
SkillSurvey will continue to offer a dental plan through UHC at no cost to you. All members will continue to have access to the UHC Options PPO 20 Network. With the Dental PPO plan, you have the flexibility to receive dental treatment from any dentist you choose, either participating or non-participating. However, you will experience cost savings by utilizing in-network dentists. Make sure to register with the UHC portal to access provider listings, track claims, and much more.
Please note, your dental deductible and out-of-pocket maximum will now reset on a PLAN YEAR basis (6/1-5/31) as opposed to CALENDAR YEAR (1/1-12/31).

VISION
VISION PLAN
Driving to work, reading a news article and watching TV are all activities you likely perform every day. Your ability to do all of these activities, though, depends on your vision and eye health. Vision insurance can help you maintain your vision as well as detect various health problems. SkillSurvey's vision insurance through UHC entitles you to specific eye care benefits. Our policy covers routine eye exams and other procedures, and provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses.
Participating vision providers are paid up to the maximum plan allowance. Participating providers must accept the maximum allowance and cannot bill any amounts in excess of the allowance.
Non-participating vision providers can be utilized; however, may result in a higher out-of-pocket cost. Members pay for the full cost of service and then can be reimbursed back a portion of the cost as outlined below.
