BASIC LIFE/AD&D INSURANCE
Life insurance can help provide for your loved ones if something were to happen to you. Help USA provides full-time employees with life insurance of 1x annual earnings up to $100,000.
Accidental death and dismemberment (AD&D) equals your Basic Life Insurance coverage amount and, under qualifying circumstances, is paid to your beneficiary in addition to the Basic Life benefit. If you become dismembered, the policy pays according to a schedule based on the dismemberment.
VOLUNTARY LIFE/AD&D INSURANCE
While Help USA provides basic life insurance, some employees may want to purchase additional coverage. This is about your personal circumstances. Depending on your needs, you may want to consider buying voluntary life insurance through Guardian. With this benefit, you are responsible for paying the full cost of coverage through payroll deductions.
- Employees may purchase 2x or 3x their annual salary to a maximum of $400,000.
- Coverage is also available to eligible spouses and dependents.
The guaranteed issue amount for employees is $200,000 and for spouses is $5,000, if enrolling in the coverage for the first time during your new hire enrollment window. Please note that if this is not your first time being eligible for voluntary life, you will be required to complete an Evidence of Insurability form and submit to Guardian for approval (instructions for submission can be found in the document linked below).
SHORT TERM DISABILITY
Short Term Disability insurance can be referred to as "paycheck" insurance. The benefits payable are a percentage of pre-disability earnings up to a maximum per month. This state-mandated benefit provides payments due to disability; payment and duration of coverage are determined by state guidelines.
- All employees are covered after thirty days of service.
- 50% of average weekly wages capped at $340 for a maximum period of 26 weeks.
LONG TERM DISABILITY
What would you do if a sickness or injury left you unable to come to work and earn your paycheck? If you become disabled from an injury or sickness, disability income benefits will provide a partial replacement for lost income. Employees are eligible for Long Term Disability after 90 days of employment. Long Term Disability pays up to 60% of your average monthly earnings up to a maximum of $6,000 per month.
Family Medical Leave Act (FMLA)
- Employees are eligible to participate after twelve (12) months of service and one thousand two hundred and fifty (1250) hours of work.
- FMLA can be used but not limited to childbirth, care of newborns, care of family members, etc.
- This is unpaid job-protected leave for a maximum period of 12 weeks
Paid Family Leave (PFL)
- Employees are eligible to participate after six (6) months of service
- PFL can be used but not limited to bond with new born, adopted or fostered child, care of family member, etc.
- Employees are paid up to 67% of their weekly wage capped at $1,131.08 for a maximum period of 12 weeks.
Bereavement Leave
- Employees are eligible to participate after ninety days of service.
- Three (3) days of paid leave in the event of death of an immediate family member.
Military Leave
- Employees who are members of the U.S. Armed forces, U.S. Armed Forces reserve, or state militia forces will be granted leaves of absence for military service, training, and other covered obligations in compliance with federal and state laws.
- Employees will be paid as per their regular rate for a maximum period of thirty (30) days.
Jury Duty
- Employees will be paid as per their regular rate if called for jury duty or as a witness:
- Full-Time employees will be paid up to a period of four (4) weeks
- Part-Time employees will be paid up to a period of two (2) weeks and are eligible to participate after ninety days of service.
