Understanding your healthcare options and determining the best plan options that fit your needs should be a high priority. We understand that navigating through your benefits package can be complex and wish to offer as much support and valuable information as possible.
BENEFITS OVERVIEW
MEDICAL PLANS: Cigna is your medical carrier. The Base, Low, and Mid plans are available for 2024.
WELLNESS: You and your spouse have access to Cigna’s MotivateMe incentive program. You can earn up to $125 in gift cards by completing various health-related activities.
TELEMEDICINE: If you are enrolled in one of the Cigna medical plans, you and your enrolled dependents will have access to virtual care. Click here for more information.
DENTAL PLANS: Cigna is your dental carrier and there are three plan options available.
VISION PLANS: Cigna is your vision carrier for 2024. The provider network transitioned from VSP to EyeMed in 2024. The claims process remains the same, however, you will need to verify with your provider that they are participating in the new Cigna Vision Network for 2024.
SPENDING ACCOUNTS: You have the opportunity to participate in a Healthcare Medical FSA, Dependent Care FSA, Commuter Transit and Parking spending accounts through OCA. You will be able to contribute pre-tax payroll deductions up to the 2024 IRS limits.
BASIC & VOLUNTARY LIFE AND AD&D: Guardian is the carrier for Life and AD&D. Help USA provides full-time employees with 1x their base salary up to a maximum of $100,000 at no cost. Supplemental Life/AD&D is available to full-time employees and dependents on a voluntary basis.
SHORT-TERM DISABILITY: Guardian will administer the NY DBL (state-mandated short-term disability). All employees are covered after 30 days of service.
LONG-TERM DISABILITY: Guardian is the carrier for Long-Term Disability. Coverage is provided for full-time employees that have completed 90 days of employment at no cost.
EMPLOYEE ASSISTANCE PROGRAM: You will have access to Evernorth Behavioral Health, Inc.'s EAP. You have access to 24/7 confidential counseling at no cost. All employees and immediate family members are eligible for up to 5 free sessions.

WHO IS ELIGIBLE?
Non-union full-time employees working 30 or more hours per week are eligible for Help USA health insurance.
New hires can elect benefits from day one of employment until the 89th day. Benefits elected during this period are effective on the 90th day of employment. Any other change in benefits will require a special enrollment window like a qualified life event or open enrollment.
DEPENDENT COVERAGE
In addition to electing coverage for yourself, you can elect to cover your eligible dependents. Your eligible dependents include your spouse, your children up to age 26 regardless of student status, marital status, financial dependence or residence and your children of any age who are mentally or physically disabled and depend on you for support.
Important: The list above describes people who are eligible to be covered as dependents under your plan. Documentation may be required at a later time. This includes, but is not limited to, birth certificates, marriage license, etc. If you have any questions please contact Human Resources.
EMPLOYEES EXPERIENCING A QUALIFIED CHANGE OF STATUS
The following Qualifying Life Events are special circumstances that enable you to change your benefits during the plan year:
Qualifying events include:
- Marriage
- Birth, adoption or placement for adoption of an eligibile child
- Divorce, legal separation or annulment of marriage
- Loss of spouse's job or change in work status (when coverage is maintained through spouse's plan)
- A significant change in your or your spouse's employment
- Death of spouse or dependent
- Loss of dependent status
- Becoming eligible for Medicare, Medicaid, or CHIP during the plan year
- Receiving a Qualified Mecial Child Support Order
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period.
If you have a Qualified Life Event during the plan year and want to make a change in your coverage, you must notify Human Resources and complete the necessary paperwork within 30 days of the occurrence of the Qualified Life Event.
