Employee Contribution Discounts/Premiums:

Wellness Discount

A $900 annual discount ($34.62 per check) will be applied to next year's medical rates if 3 requirements are completed and submitted to Virgin Pulse by December 1st of the current plan year. An additional $200 annual discount ($7.69 per check) will be deducted from those rates if you are within range or have improved biometric screening results from a prior year biometric screening for at least 2 of the 4 tested categories. *Opportunity to take targeted courses for credit as well.

Spousal Premium

A $2,400 annual charge ($92.30 per check) will be added to the medical rates above if your spouse has medical benefits available to them with an employer, but you choose to add them as a dependent on your medical plan. A Spousal Waiver form is available for those whose spouse is not eligible for medical coverage elsewhere. The premium will be added to anyone that has a spouse on the medical plan if a waiver form is not submitted.

Smoker's Premium

A $600 annual charge ($23.08 per check) will be added to the medical rates listed in the table above if the employee is a smoker and enrolled in the medical plan.
